Tomball Regional Medical Center Home Health is a hospital based home health agency that provides an array of therapeutic and support services. Our goal is to provide each patient quality personalized care while maintaining independence at home. We work closely with each patient's physician to ensure that all medical treatment needs are met. We communicate with the patient's family to manage patient's needs. This program gives patients the ability to leave the hospital and return home sooner.
Our staff of experienced caregivers includes professional nurses, physical therapists, occupational therapists, speech therapists, social workers and home care aides. A registered nurse or physical therapist visits patients in their home and develops a plan of care with the patient's physician. A patient's doctor is notified of any change in the patient's condition or needs, to facilitate appropriate adjustment to the plan of care.
TRH Home Health is accredited and regulated by federal and state agencies. All of our home health staff are thoroughly screened, including background checks. Medicare, Medicaid, and commercial insurance plans usually cover TRH Home Health services. We provide services without regard to race, color, sex, national origin, age or handicap.
Although a physician order is required to initiate home care services, referrals and additional information can be obtained through your hospital discharge planner, your physician's office staff, a medical social worker or TRH Home Health. You can be directly referred to TRH Home Health without having to be admitted to the hospital.

When a person is under the care of a physician, has special care needs, or becomes homebound, then home health care may be considered medically necessary. A patient does not need to be totally confined to home to qualify for home care, but a person who receives home health care is generally unable to leave home without considerable and taxing effort.
